-Name of person the gift is for and contact info (phone # or email address)
-Who the gift is from ( ie. just yourself or a group or family)
-Occasion gift is celebrating
-where to send the gift and introductory letter ( ie return email to you or directly to the recipient of the gift by email or Canada Post)
